The Registration Form is available in portable document format (.pdf) or as a Word document (.doc).

Registration Fees

The registration fee covers all conference sessions on Friday and Saturday, as well as dinner Friday evening and lunch on Saturday. Refreshments will be provided throughout the conference.

$60	Early Registration (postmarked by March 15)
$75	Regular Registration (postmarked after March 15)
$30	Early Student Registration (postmarked by March 15) include student institution and student ID number
$45	Regular Student Registration (postmarked after March 15) include student institution and student ID number

Payment

Please submit payment by a check made payable to Middle Tennessee State University. Regrettably, we are unable to accept credit card payments.

Cancellation and Refund Policy

All cancellations must be submitted in writing to the address on this form. Cancellations postmarked by March 21 will receive a full refund. No refunds after March 21, 2008.
